The employee meets with the ElderCare Specialist to assess the needs of the caregiver in a formal interview. This assessment consists of collecting the following:
All problems and concerns for both the older adults and the caregiver
Home safety concerns and the need for home modification
The appropriate level of care needed: In Home Care or a Senior Living Community
Additional resources needed to assure peace of mind for the entire family
This information assists in the development of a Care Plan by the ElderCare Specialist to determine the needs of the caregiver/employee and for their loved one.
